培美曲塞二钠联合顺铂治疗二线晚期非小细胞肺癌的临床观察(2)

本组15例患者采用培美曲塞二钠联合顺铂治疗二线NSCLC的近期疗效有效率6.6%(1/15),临床获益率60%(9/15),其中有效率低于文献报道,考虑与患者经历多个周期化疗,大部分患者经历分子靶向治疗后疾病进展,又采用培美曲塞治疗有关,中位生存期8.0个月,疾病无进展中位生存时间2.3个月,与文献报道相似。其毒性反应主要疲乏无力,占53.3%。其次为白细胞下降,占46.6%,其中III度下降6.6%。无IV度血小板下降。恶心、呕吐占33.3%。未出现过敏反应及皮疹。脱发、肝肾功能异常,便秘发生几率低。因治疗过程中口服叶酸及应用维生素B12,贫血发生率较低。与紫杉醇及吉西他滨方案比较,其毒性反应轻,部分患者KPS评分提高,耐受性好。
综上所述,培美曲塞二钠联合顺铂是治疗二线NSCLC较好化疗方案,其毒性作用可以耐受。因本组报道病例数较少,其数据可能有些偏差。待大规模临床试验进一步证实。
